STATE OF WASHINGTON REQUIRED DISCLOSURE STATEMENT

Licensure, Training, and Experience  
I have been a practicing psychotherapist since 1990. I work with individuals, couples, and groups, and am currently licensed by the State of Washington (#LH00009532).  I received a B. S. Degree in psychology as well as a Masters Degree in Clinical Counseling at Southern Oregon University.  I have had training and have volunteered in Southern Oregon as a Court Appointed Special Advocate for children and families.  I have attended trainings and seminars for psychotherapy with individuals, couples, and groups through the American Psychoanalytic Association, Family Services in Seattle, the Washington Institute, Northwest Family Development Center, Puget Sound Group Psychotherapy Network, and Northwest Alliance for Psychoanalytic Study. I have worked as an outpatient therapist for Community Psychiatric Clinic in Seattle  from  2001–2006. I am currently training in Seattle based study groups with Allan Schore on neurobiology and attachment theory in clinical practice and with Stan Tatkin on the neurobiology of relationships in couple’s therapy.

Approach and Goals of Therapy
I believe that when someone is seeking professional help in therapy that they already have some pretty good ideas about what they want.  My job then is to listen carefully and together we will discuss goals and how those goals might be realized through therapy. While my training and clinical experience have provided me with many ways to approach therapy, I have learned that the most effective work centers around attention to current life situations and discussion of the impact of current and past relationships.  Whether the work is on personal issues, life transitions, or relationships themselves, how one finds oneself in relationship to others will come up again and again.  My role as therapist is to help you become the informed expert of your own therapy, and to his end I remain available to open discussion of the therapy at any time.

Confidentiality
I treat as confidential the fact that you are receiving therapy as well anything that you share with me, except in the following cases:
- if you sign a release for me to exchange information with another person or agency
            - if I believe there is a danger of you harming yourself or another person
- if someone in your custody or care is in danger of being abused or has been abused
- if I receive a court order requiring that I release my records

To enhance my work with clients I occasionally consult with a Licensed Clinical Supervisor and other professionals.  When doing so I maintain strict confidentiality regarding the identity of client(s).  

Fees and Appointments
My fees are:
    $90 per 50 minute individual session
    $135 per 75 minute couples session
    $30 per 90 minute group session  

This fee is payable at each session unless other arrangements are mutually agreed upon.  This same fee applies prorated to the ¼ hour for client initiated phone calls longer than 5 minutes as well any adjunct requests by a client, i.e.: writing a letter on clients behalf.

There is no charge for appointments that are canceled with 24 hours notice.

(206) 306-9030.  I listen to voice mails each week day and return requested calls as soon as possible.  If you have an emergency that cannot wait, please call the Crisis Clinic at (425) 258-4357 or (206) 461-3222, or call 9-1-1.

If I am out of town my telephone message will have the name and phone number of a colleague who will be available for telephone support.
